In order to register you need to live within our catchment area – please call us if you wish to confirm that you live within this boundary. If you live outside this area, your registration may be rejected (This does not apply to UAL students)
If you would like to register with us, please email at firstname.lastname@example.org and we will send you an online registration form.
We will contact you to acknowledge receipt of your registration request. To complete your registration we ask all patients to provide proof of address. If you are unable to provide this documentation at the time of registration, we will require you to provide this within one month of your registration.
As a security measure all information provided on this form will be sent as a secure, encrypted email.
