In order to register you need to live within our catchment area – please call us or use the check your postcode map on this page if you wish to confirm that you live within this boundary. If you live outside this area, your registration may be rejected (This does not apply to UAL students)

Any registration forms submitted after our cut off time will be processed the next working day.
Cut off times are:
- MONDAY & WEDNESDAY – 19:30
- TUESDAY, THURSDAY & FRIDAY – 18:00
- SATURDAY – 12:00
We will contact you to acknowledge receipt of your registration request. To complete your registration we ask all patients to provide proof of address. If you are unable to provide this documentation at the time of registration, we will require you to provide this within one month of your registration.
As a security measure all information provided on this form will be sent as a secure, encrypted email.
